# Phiếu thu thập thông tin

## **Tổng Quan Phiếu Thu Thập Thông Tin Khách Hàng**

Phiếu thu thập thông tin khách hàng được thiết kế để thu thập các thông tin cơ bản và quan trọng về khách hàng trước khi họ tiến hành khám bác sĩ. Điều này giúp bác sĩ có cái nhìn tổng quát về tình trạng sức khỏe, lịch sử bệnh tật, và các yếu tố cá nhân khác của khách hàng, từ đó hỗ trợ quá trình chẩn đoán và điều trị hiệu quả hơn.

**Các tính năng chính:**
